Application
Please complete an Application for each student. If you wish, you may download a copy of our application packet. Please type or print clearly. A non-refundable application/testing fee of $50 must be attached to each application. It is the parent's responsibility to ensure that all applications are complete.All of the following must be received in order to be considered for admission:
- Grade Appropriate Application
- Non-Refundable Application/Testing Fee of $50 per child.
- Birth Certificate- Include a copy of the birth certificate.
- Baptismal/Confirmation Certificates-Catholic applicants must include a copy of the baptismal certificate as well as a copy of the First Holy Communion and Confirmation certificates, as applicable.
- Student Evaluation Form (to be completed by child's current teacher) - Request that your child's teacher(s) complete the form(s) and submit with your child's transcript to Canyon Heights Academy. Please use the appropriate grade level form.
- Transcript Release- Please sign and submit the Transcript Release Form to your child's current school so that your child's complete records will be transferred to Canyon Heights Academy. Transcripts are needed for elementary and middle school grades as applicable. It is the parent's responsibility to ensure that transcripts are forwarded in a timely manner.
- Current Photo- Please attach a current photo of your child to the application.
- Tuition Assistance- If you are interested in applying for Tuition Assistance, instructions are under the tuition assistance tab. Please contact the admissions office for current tuition assistance deadlines.
Student Assessment
All applicants must be tested prior to admissions and placement in order to ensure an adequate fit between the applicant and the academic and social expectations of the individual classrooms. All assessments are scheduled by appointment and conducted in a one-on-one or small group setting by the appropriate member of the school faculty. Preschool assessments usually take between 15 to 30 minutes. Lower school assessments usually take between 45 and 90 minutes. Middle school assessments usually take 90 to 120 minutes.
Family Interview
After the completion of all testing and upon reception of all transcripts and recommendations, a family interview with the Principal will be scheduled. This interview is scheduled by the Recruiting Director and usually takes 30 minutes to an hour.
